Zusammenfassung

In dieser Übersicht sollen diagnostische Kriterien für die verschiedenen Demenzen und die leichte kognitive Störung dargestellt werden. Es zeigt sich, dass bei den derzeitigen therapeutischen Möglichkeiten bei degenerativen Demenzen nach Ausschluss einer symptomatischen Demenz ein kontrollierter Behandlungsversuch mit Cholinesterasehemmern oder Memantine aufwändiger Diagnostik überlegen ist. Dies wird sich ändern, wenn für die verschiedenen degenerativen Demenzen und für die zerebrale Mikroangiopathie wirksamere Medikamente zur Verfügung stehen. Die klinischen Bilder der verschiedenen Demenzsyndrome werden kurz dargestellt.

Summary

This review presents diagnostic criteria for the different dementia syndromes and mild cognitive impairment. It is being claimed that in view of the current pharmacological interventions and after exclusion of symptomatic dementia, a controlled trial with cholinesterase inhibitors or memantine is more efficient than elaborate diagnostics. Efficiency of differential diagnosis will increase when drugs are available that specifically improve the different degenerative dementias and vascular dementia. Clinical pictures of the various dementia syndromes are briefly described.
