Zusammenfassung
Akute Porphyrien sind zwar selten, führen aber häufig zu fehldiagnostizierten und dramatisch verlaufenden Krankheitsbildern. Verschiedene Krankheitsbilder können durch die Kombination internistischer, psychiatrischer und neurologischer Symptome imitiert werden. Es wird über eine 40-jährige Patientin berichtet, die mit einer subakut aufgetretenen Tetraparese in unserer Klinik vorgestellt wurde. In den vorangegangenen Monaten war die Patientin mehrmals wegen abdomineller Beschwerden in einer internistischen und wenig später wegen deliranter Symptome mit Halluzinationen auch in einer psychiatrischen Klinik behandelt worden. Die typische Trias einer motorischen axonalen Neuropathie, abdomineller Beschwerden und psychiatrischer Symptome führte über die Bestimmung der Pophyrinmetaboliten im Harn rasch zu der Diagnose einer akuten intermittierenden Porphyrie. Die Prognose akuter Porphyrien hat sich durch Einblicke in die Pathogenese dieser Erkrankungen deutlich verbessert. Die Bestimmung der Enzymaktivitäten des Porphyrinstoffwechels ermöglicht auch im beschwerdefreien Intervall eine Diagnosestellung. Im Fall einer positiven Familienanamnese kann ein molekulargenetisches Mutationsscreening asymptomatische Familienmitglieder mit erhöhtem Risiko, an einer Porphyrie zu erkranken, identifizieren.
Summary
Acute porphyrias are rare, but often misdiagnosed and may take a dramatic clinical course. The combination of various internal, psychiatrical and neurological symptoms can mimic different other diseases. We report a 40-year-old female patient who was admitted with a subacute tetraparesis. During the last 2 months the patient was treated several times because of abdominal pain and just before admission to our clinic in a psychiatric hospital because of acute mental changes and hallucinations. The typical combination of abdominal pain, motor neuropathy and psychiatric symptoms confirmed by increased amounts of porphyrines and their precursors, led us to promptly diagnose acute intermittent porphyria. Better knowledge about the pathogenesis has clearly improved the prognosis of acute porphyria. In remission, measurement of enzyme activities or mutation screening can be the only diagnostic verification. A mutation screening for family members should be conducted to identify symptom-free carriers, especially in cases of a positive family history.
