Zusammenfassung
Wir untersuchten die Einjahresprävalenz von Schmerzen bei 157 konsekutiv stationär aufgenommenen Patienten mit klinisch eindeutiger Multipler Sklerose, 90 Frauen und 67 Männer im Alter zwischen 19 und 85 Jahren und einem Behinderungsgrad (EDSS) zwischen 1,0 und 8,5. Die Datenerfassung erfolgte persönlich und standardisiert mit Klassifizierung der Schmerzsyndrome, wobei in die Auswertung nur schwere Schmerzen (VAS mindestens 4/10) aufgenommen wurden, die innerhalb der vergangenen 12 Monate mindestens dreimalig und/oder über einen Zeitraum von einer Woche kontinuierlich aufgetreten waren. 61% der 157 Patienten litten an 176 Schmerzsyndromen: am häufigsten wurden beklagt Kopfschmerzen (40%), schmerzhafte Missempfindungen (19%), Kreuzschmerzen (17%) und schmerzhafte Spastik (11%). 12% der Schmerzen wurden als schlimmstes MS-Symptom benannt, bei 68% wurde die Versorgung durch vorbehandelnde Ärzte als unbefriedigend angesehen. Dies galt überraschenderweise auch für den am häufigsten diagnostizierten Schmerz, die Migräne, wo klare Therapieempfehlungen existieren. Somit besteht dringender ärztlicher Handlungsbedarf zu einer verbesserten Wahrnehmung von Schmerzen bei MS-Patienten.
Summary
We evaluated the 1-year prevalence of pain syndromes and quality of care among 157 consecutive multiple sclerosis (MS) inpatients (90 f, 67 m) aged 19–85 years, with extended disability status scores of 1.0–8.5 and clinically definite MS. In a standardized questionnaire, only severe pain (pain intensity on visual analog scale of at least 4/10) was documented and classified which had occurred more often than three times or lasted longer than 1 week within the last year. Of 157 patients, 61% reported 176 pain syndromes: most frequent were headaches (40%), dysesthetic limb pain (19%), back pain (17%), and painful spasms (11%). Twelve percent of the pain syndroms were classified as worst symptom of MS, and in 68% insufficient care by the physicians consulted was reported. This was even true for the most frequent pain, migraine, in which clear treatment recommendations exist. There is thus an urgent need for physicians to keep this problem in mind when treating MS patients.
