Pathologische Frakturen

Pathologische Frakturen sind definiert als Frakturen, die an einem aus systemischen oder lokalen Gründen krankhaft vorgeschädigten und mechanisch vermindert belastbaren Knochen eintreten. Ihre Häufigkeit beträgt ohne die Osteoporose ca. 5% aller behandelten Frakturen. Wichtigste diagnostische Aufgabe ist es, das Vorliegen einer pathologischen Fraktur überhaupt zu erkennen, um dann eine ggf. interdisziplinäre ursachenspezifische Therapie einzuleiten. Hierfür werden diagnostische und therapeutische Algorithmen vorgestellt, geordnet nach Osteopathien, Konstitutionsanomalien, myelogenen Osteopathien, entzündlichen Knochenerkrankungen, primären und sekundären Knochentumoren und posttraumatischen und postoperativen Ursachen. Diese Algorithmen sollen den Unfallchirurgen in die Lage versetzen, die entsprechende konservative oder operative Therapie im Einzelfall festzulegen und die vielfältigen Möglichkeiten von Resektion und Rekonstruktionen (Osteosynthese, alloplastischer Gelenkersatz, Knochentransplantation u. a.) situationsadaptiert einzusetzen.

Pathological fractures are defined as fractures that occur in patients with weakened bone due to systemic and local diseases. If osteoporosis is excluded, the frequency amounts to 5% of all fractures treated. The main diagnostic aim is to observe the pathological nature of the fracture and to perform interdisciplinary, causal therapy. For this, diagnostic and therapeutic algorithms are demonstrated, arranged according to osteopathies, constitution anomalies, myelogenous and inflammatory bone diseases, primary and secondary bone tumors and posttraumatic/postoperative disorders. These algorithms should help the trauma and orthopedic surgeon to select the proper conservative and operative therapy and to use all resection and reconstruction possibilities, such as osteosynthesis, alloarthroplasty and bone transplantation.
