Zusammenfassung
Die Anzahl der Niedrigenergietraumen im Alter mit folgenden Beckenringfrakturen steigt an im Gegensatz zu den Hochenergietraumen mit Beckenverletzungen bei jüngeren Menschen. Die Behandlung der geriatrischen Frakturen am Becken kann bei einem Teil der Patienten je nach Beschwerdesymptomatik konservativ mit analgetischer Therapie und krankengymnastisch assistierter Mobilisation durchgeführt werden. Wenn die Beschwerden keine adäquate Mobilisierung zulassen, dann ist eine operative Stabilisierung indiziert. Der hintere Beckenring kann oft minimal-invasiv transiliosakral verschraubt werden. Bei zusätzlicher Instabilität mit Beschwerden am vorderen Beckenring besteht neben der invasiven Plattenosteosynthese die Option der perkutanen, intramedullären Stabilisierung mit einem Kunststoffpolymer.
Abstract
The number of low energy traumas in older people with subsequent pelvic ring fractures is increasing in contrast to high energy traumas with pelvic injury in younger people. Geriatric pelvic fractures can be treated conservatively with analgesics and physiotherapy-assisted mobilization, depending on the symptoms. If physical complaints do not allow adequate mobilization, surgical stabilization is indicated. It is often possible to stabilize the dorsal pelvic ring with transiliosacral screws. If additional instability associated with anterior pelvic ring complaints is prevalent, stabilization of the anterior ring can be achieved by invasive osteosynthesis using a plate or percutaneously by implanting an intramedullary plastic polymer.
