Zusammenfassung
Bei Versagen der konservativen Therapie oder eines primären operativen Eingriffs mit Rezidivinstabilität des Akromioklavikular- (AC-)Gelenks müssen Alternative Verfahren gewählt werden. Die konservative Therapie ist meist wenig Erfolg versprechend. Heute hat sich neben dem Weaver-Dunn-Verfahren und seiner Modifikationen eine Reihe von Eingriffen den Weg in die Literatur und die Ooerationssäle gebahnt, deren Prinzip darin besteht, eine primär hohe Stabilität durch ein mehr oder weniger rigides Implantat zu erreichen und dann eine biologische Augmentation mittels Sehnentransplantat zu erzielen. Diese Eingriffe werden nun zunehmend auch arthroskopisch oder arthroskopisch assistiert durchgeführt.
Abstract
Failed conservative treatment or surgical procedures for acute AC joint seperations create the demand for revision or salvage procedures to successfully treat persistent AC joint instability. Besides the classic Weaver-Dunn procedure and its modifications lately new procedures combining a more or less rigid implant for primary stability with a biologic augmentation using an autologous tendon graft have been developed. These procedures are performed arthroscopically or arthroscopically assisted.
