Zusammenfassung
Die adäquate Versorgung einer distalen Tibiafraktur eines multimorbiden Patienten mit ausgesprochen schlechten Haut- und Weichteilverhältnissen, verminderter peripherer vaskulärer Versorgung sowie osteoporotischen Knochenbedingungen ist nur schwer realisierbar. In dieser Kasuistik präsentieren wir 2 Patientinnen mit distalen Tibiafrakturen. Aufgrund der Extremsituationen der Patienten erschien keines der konventionellen Therapieverfahren (Gips, Plattenfixation, Fixateur externe, anterograder Tibiamarknagel) erstrebenswert. Um dennoch eine suffiziente Frakturheilung gewährleisten zu können und diese Patienten vor einer Amputation zu bewahren, wählten wir den außergewöhnlichen Behandlungsansatz der retrograden Tibiamarknagelung unter Einschluss des oberen und unteren Sprunggelenks.
Abstract
The appropriate treatment of a distal tibia fracture in multimorbid patients with poor soft tissue and skin conditions, poor peripheral blood supply and osteopenic bone is challenging. In this case report we present two patients with these characteristics. Given these exceptional conditions none of the conventional therapies (e.g. cast, internal plate fixation, external fixation, anterograde intramedullary nailing) appeared feasible. To achieve sufficient fracture healing, we chose an unconventional treatment approach of retrograde tibial nailing with inclusion of the ankle and subtalar joint.
