Diagnostik und Therapie von radiokarpalen Luxationsfrakturen

Zusammenfassung

Radiokarpale Luxationsfrakturen sind, im Gegensatz zu „gewöhnlichen“ extra- oder intraartikulären Radiusfrakturen, seltene Verletzungen. Die Anzahl der Publikationen zu diesem Thema ist gering. Nichtsdestotrotz ist die Kenntnis dieser Verletzungen für den unfallchirurgisch tätigen Arzt wichtig, da aufgrund der Verletzungsschwere häufig eine zügige operative Intervention notwendig ist und eine alleinige Ruhigstellung nach geschlossener Reposition vielfach nicht ausreicht. In einigen Fällen können zudem Handwurzelverletzungen vorliegen. In derartigen Fällen müssen dann nicht nur die distale Radiusfraktur und die Luxation reponiert, sondern auch die Handwurzelverletzung versorgt werden. Bei Fehlinterpretation oder Nichterkennen der Verletzung bzw. nicht ausreichender Reposition und Stabilisierung drohen Gelenkinkongruenzen und arthrotische Veränderungen. Von besonderem Interesse ist die Rekonstruktion des radiokarpalen Bandapparats.

Abstract

In contrast to the common intra- or extra-articular fractures of the distal radius, radiocarpal fracture dislocations are rare injuries. Concerning this issue, only a small number of publications can be found. Nevertheless, it is important to be informed about this injury since prompt operative treatment is often required and immobilization alone will not be sufficient. Sometimes, radiocarpal fracture dislocations are combined with carpal injuries. In such cases, both the radiocarpal dislocation and carpal injury have to be treated. Diagnostic difficulties can lead to misinterpretation or underdiagnosis. Insufficient reduction and fixation may result in joint incongruity and subsequent osteoarthritis. Reconstruction of the radiocarpal ligaments is a substantial part of operative treatment.
