Zusammenfassung
Das vorrangige Ziel der Behandlung von Mittelhandfrakturen ist die Wiederherstellung der Greiffunktion der Hand. Ob eine konservative Therapie möglich ist, muss sowohl anhand radiologischer Kriterien als auch anhand des Ausmaßes der klinischen Fehlstellung individuell entschieden werden. Für die operative Behandlungen von Frakturen im Kopf- und Schaftbereich haben sich weichteilschonende Schrauben- und Plattenosteosynthesen bewährt sowie insbesondere für die Halsfrakturen der Randstrahlen die intramedulläre Markraumschienung mit Drähten. Die besonders instabilen Basisfrakturen des ersten Mittelhandknochens werden mit dem Ziel der Form- und Gelenkflächenwiederherstellung in frakturadaptierter Technik regelhaft operativ behandelt. Sowohl bei der konservativen als auch nach der operativen Therapie ist eine frühfunktionelle Weiterbehandlung zur Vermeidung von Gelenkfunktionsstörungen anzustreben.
Abstract
The major goal in the treatment of metacarpal fractures is to restore the normal function of the hand. Radiological criteria and the clinical extent of displacement should be individually considered when taking the decision for or against conservative treatment. Internal fixation techniques must protect soft tissue structures. Small screws and plates have proven effective for head and shaft fractures, whereas intramedullary splinting is favoured for neck fractures. In instable and displaced fractures of the base of the first metacarpal, surgery is regularly performed to restore the bony shape and articular surface. To prevent functional impairments, early mobilization is desirable both during conservative treatment and following internal fixation.
