Zusammenfassung
Das Chondrosarkom stellt das zweithäufigste mesenchymale Malignom des Knochens dar. Die Differenzierung in unterschiedliche Formen erfolgt aufgrund klinischer, radiologischer und histologischer Gesichtspunkte. Über den Fall eines intrakortikalen Chondrosarkoms berichteten erstmals Babinet et al. 2003 [2]. Bei einem 59-jährigen Patienten diagnostizierten wir im Rahmen von Staging-Untersuchungen aufgrund eines Plattenepithelkarzinoms des Oropharynx eine zunächst metastasenverdächtige Läsion am rechten distalen Femur. Nach marginaler Tumorresektion und Verbundosteosynthese wurde histologisch ein hochmalignes intrakortikal gelegenes dedifferenziertes Chondrosarkom diagnostiziert. Dabei handelt es sich bei dieser ohnehin eher seltenen Erkrankung hinsichtlich der Lokalisation um eine ausgesprochene Rarität. Die im vorliegenden Fall durchgeführte Therapie ohne vorherige bioptische Abklärung entspricht nicht den Standards der muskuloskelettalen Tumorchirurgie und kann im ungünstigsten Fall eine signifikante Prognoseverschlechterung bedeuten. Diskutiert werden differenzialdiagnostische Überlegungen und ihre Implikationen für die nachfolgende Therapie.
Abstract
Chondrosarcoma is the second most frequent mesenchymal malignant tumour of the bone. Classification of this kind of tumour is made by clinical, radiological und pathological means. A case of an intracortical chondrosarcoma was first reported by Babinet et al. 2003 [2]. During the staging examination of a 59-year-old patient referred to our clinic because of a squamous cell carcinoma of the oropharynx, we also found a highly malignant intracortical dedifferentiated chondrosarcoma of the distal femur shaft. Due to the primary assumption of bone metastasis of the oropharynx tumour, marginal extralesional tumour resection was performed followed by composite osteosynthesis. Considerations on differential diagnosis and their implications for further therapy are discussed.
