10 Frakturen in 21 Jahren

Zusammenfassung

Bei einer 78-jährigen Patientin traten ab dem 57. Lebensjahr 10 Spontanfrakturen auf, die zur Immobilität führten. 21 Jahre später wurde die zu Grunde liegende Osteomalazie diagnostiziert. Ursache der Osteomalazie war eine oligosymptomatische Sprue mit Malabsorptionssyndrom. Neben einer glutenfreien Kost ist die Substitution von Kalzium und Vitamin D erforderlich, bis Kalzium, alkalische Phosphatase (AP) und Vitamin D sich normalisieren. Zur Vermeidung weiterer Frakturen erfolgt bei jeder Fraktur ohne adäquates Trauma eine osteologische Diagnostik. Eine erhöhte AP deutet auf eine Osteomalazie hin, woraus sich andere diagnostische und therapeutische Grundsätze ergeben als bei Osteoporose.

Abstract

A 78 year old woman had suffered ten spontaneous bone fractures, the first occurring when she was 57 years old. She is now wheel-chair bound. After 21 years, the underlying osteomalacia due to oligosymptomatic celiac disease with malabsorption was diagnosed. The treatment consists of a gluten free diet and substitution of calcium and vitamin D until there is a normalisation of calcium, vitamin D and alkaline phosphatase. Any spontaneous fracture deserves a careful search for metabolic bone disease. An elevation of alkaline phosphatase indicates osteomalacia rather than osteoporosis.
