Zusammenfassung
Die Infektion des Hüftgelenks infolge eines Psoasabszesses ist ein seltenes Ereignis. In der Literatur sind lediglich zwei Fälle beschrieben. Die Klinik des Psoasabszesses ist durch unspezifische Rückenschmerzen und Hüftbeschwerden gekennzeichnet, wodurch sich die meist verspätete Diagnosestellung erklären lässt. Richtungweisend können MRT und Hüftpunktion sein. Die Behandlung der Wahl ist die zügige chirurgische Sanierung mit Abszesseröffnung und resistenzgerechter Antibiotikatherapie. Nur selten ist eine perkutane Drainage ausreichend. Bei Infektion der Hüfte kann häufig die Resektion des Gelenks nicht vermieden werden. Nach vollständigem Rückgang der Entzündungsparameter und sicherer, durch wiederholte Hüftpunktionen nachgewiesener Keimfreiheit, kann ggf. ein prothetischer Gelenkersatz erfolgen, wobei hier die Entscheidung individuell zu treffen ist, da auch eine Girdlestone-Hüfte durchaus befriedigende Resultate hinsichtlich Schmerzfreiheit und Mobilisation erbringen kann.
Abstract
Infection of the hip joint following psoas abscess is uncommon. In fact, only two cases have been reported in the literature. The clinical aspect of psoas abscess is characterised only by unspecific back pain and hip pain, which are responsible for delayed diagnosis. MRT and culture of joint aspirate can permit early diagnosis. The preferred treatment is immediate surgical intervention with debridement, drainage and antibiotic therapy. Percutaneous drainage of psoas abscess alone only rarely is sufficient. In cases of infected hip joint, resection arthroplasty often cannot be avoided. Following complete decline of inflammatory parameters and certain absence of bacteria confirmed by repeated joint aspiration, prosthetic joint replacement is possible. This decision should be made on an individual basis, because Girdlestone hip also can lead to a satisfactory result concerning pain relief and functional outcome.
