Zusammenfassung
Frakturen des Capitulum humeri sind selten. Trotz adäquater traumatologischer Therapie kommt es häufig zu Bewegungseinschränkungen, avaskulären Nekrosen des Frakturfragments und neurologischen Störungen (Traumatisierung von Nn. radialis, medianus und cutaneus antebrachii lateralis). Ziel der vorliegenden retrospektiven Studie war es unter Berücksichtigung dieser Komplikationen kurz- und mittelfristige Ergebnisse von Capitulumfrakturen Typ I nach Bryan und Morrey, die über einen ventralen Zugang nach Henry mit Doppelschraubenosteosynthese versorgt wurden, zu untersuchen.
Die Patienten (3 Frauen, 4 Männer, 21–85 Jahre) wurden nach 10–65 Monaten klinisch (Morrey-Score), radiologisch und neurologisch untersucht. Bei allen Patienten kam es zu einer knöchernen Konsolidierung der Fraktur, in keinem Fall konnte ein neurologisches Defizit nachgewiesen werden; 5 der 7 Patienten erreichten ein sehr gutes oder gutes Ergebnis im Morrey-Score.
Der ventrale Zugang nach Henry stellt bei dislozierten Capitulum-humeri-Typ-I-Frakturen eine geeignete Option für eine anatomisch korrekte Osteosynthese dar. Avaskuläre Nekrosen und neurologische Komplikationen traten im untersuchten Patientengut nicht auf. Das zu erwartende Bewegungsdefizit ist für die Mehrzahl der Patienten in einem funktionell gering bedeutsamen Ausmaß.
Abstract
Capitellar fractures are rare. Despite adequate surgical treatment, patients suffer frequently from limited range of motion, osteonecrosis, and neurological deficits. The purpose of this retrospective study was to evaluate short- and medium-term surgical results in seven patients who had type I capitellar fractures according to Bryan and Morrey. All patients were treated via an anterior approach, according to Henry, and open reduction and internal fixation with two screws.
Seven patients were examined clinically, neurologically, and radiologically after 10–65 months. The fracture healed in all patients. None of the patients had a neurological deficit. According to the Morrey Score, five of seven patients had an excellent or good functional result.
Open reduction and internal fixation with two screws via the Henry approach is a suitable option for surgical treatment for type I capitellar fractures.
