Zusammenfassung
Die operative Stabilisierung von osteoporotischen Wirbelkörperfrakturen stellt die Ausnahme dar und wird im Wesentlichen bei den seltenen Frakturen mit neurologischen Defiziten durchgeführt. Wir präsentieren eine Verbundosteosynthese bei dorsaler Instrumentierung einer osteoporotischen LWK-1-Fraktur als „salvage procedere“ nach Dislokation einer konventionellen dorsalen Instrumentierung bei einer 76-jährigen Patientin mit inkompletter Querschnittsymptomatik. Eine zusätzliche ventrale Stabilisierung mit Wirbelkörperersatz kam wegen des Alters der Patientin und im Verlauf stattgehabter Pneumonie nicht infrage. Während der Revisionsoperation führte auch die Zementierung der Pedikelschrauben zu keiner ausreichenden Stabilisierung, sodass zusätzlich dorsal Palacos-Zement im Sinne einer Verbundspondylodese angelagert wurde.
Abstract
The surgical treatment of osteoporotic vertebral compression fractures is rarely necessary and is mostly done in fractures with neurological deficit. We present a case of compound osteosynthesis in an osteoporotic fracture of the first lumbar vertebra as a “salvage procedure” after a dislocation of a dorsal internal fixator in a 76 year old female sustaining incomplete paraplegia. In this case, an additional ventral stabilization using a vertebral body replacement was not practicable because of old age and previous pneumonia. We performed an additional compound osteosynthesis using bone cement because of insufficient stability when only using cement for augmentation of the pedicle screws.
