Zusammenfassung
Das Osteosarkom stellt nach dem Plasmozytom die zweithäufigste maligne Neoplasie am Knochen dar. Seine Inzidenz liegt bei ca. 150 Neuerkrankungen/Jahr in Deutschland. Dabei sind v. a. Kinder und Jugendliche betroffen. Die häufigste Lokalisation sind die langen Röhrenknochen, das Sakrum ist nur in 1% der Fälle betroffen.
Im Gegensatz zu Tumoren an den Extremitäten führen Beckentumoren je nach Lokalisation häufig erst spät zu klinischer Symptomatik, sodass zum Zeitpunkt der Diagnose häufig schon ein fortgeschrittenes Krankheitsstadium vorliegt. Die Therapie des Osteosarkoms beinhaltet im deutschsprachigem Raum neben der chirurgischen Resektion die neoadjuvante Chemotherapie nach der kooperativen Osteosarkomstudie (COSS).
Anhand von 2 Fallbeispielen eines zentralen Osteosarkoms mit Befall des Os sacrum sollen Diagnostik, Behandlungsstrategie und Komplikationen dargestellt und diskutiert werden.
Abstract
Primary malignant neoplasms of the bone are rare. One of the most common bone tumors is osteosarcoma, most often localized in the metaphysis of adolescent long bones.
The sacrum is concerned in only about 1%. Primary bone sarcomas of the pelvis are usually recognized after they have grown to a considerable size. Osteosarcoma patients from Germany, Austria, and Switzerland are treated by neoadjuvant chemotherapy combined with complete surgical excision according to the Cooperative Osteosarcoma Study Group (COSS). Based on a review of the literature, diagnosis, treatment, and complications of two cases of sacrum osteosarcoma are introduced.
