Behandlungsergebnisse nach operativer Versorgung kindlicher Kreuzbandverletzungen

Zusammenfassung

In der Zeit vom 01.01.1994 bis zum 31.12.2000 wurden in unserer Klinik 45 Kinder aufgund von Rupturen des VKB bei offenen Wachstumsfugen operativ versorgt. Kreuzbandverletzungen bei Kindern mit noch offenen Wachstumsfugen sind selten, führen jedoch ohne Rekonstruktion des vorderen Kreuzbandes zu einer hohen Anzahl von Verletzungen der Menisken und des Knorpels sowie Auslockerung der Tertiärstabilisatoren.

Daher führen wir in unserer Klinik bei Patienten mit tibialem knöchernem Ausriss die Refixation durch.Bei intraligamentärer Ruptur führten wir entweder die Naht nach Marschall oder – in den letzten Jahren – die Ersatzoperation des vorderen Kreuzbandes mittels der Hamstringsehnen in der Trans-Fix-Technik durch. Bislang zeigten sich keine Störungen der Wachstumsfugen,wobei das Skelettwachstums bei allen Patienten noch nicht abgeschlossen ist.Bei den Nachkontrollen fanden wir nach knöcherner Refixation ausnahmslos stabile Kniegelenke, ebenso bei den Ersatzoperationen mit den Hamstringsehnen,wobei die Zuordnung beim IKDC-Score Gruppe A und B betraf und somit ein gutes und sehr gutes Resultat reflektiert.Bei den Marschall-Nähten fanden sich ausnahmslos unbefriedigende Ergebnisse. Daher können wir den Ersatz des vorderen Kreuzbandes in der Trans-Fix-Technik bei offenen Fugen zunächst in prospektiven Studien empfehlen, nicht jedoch die Naht nach Marschall.

Abstract

Between 01.01.1994 and 31.12.2000 we have operated 45 patients in our clinic with open growth plates.Crucuiate ligament injuries are seldomly found by children with open growth plates.Nevertheless, if the injury is left untreated it is liable to injure the meniscoid and the cartilage severely and may even lead to a loosening of the tertiary stabilizers.

Therefore we do refixation of anterior cruciate ligament for tibial boney rupture and Refixation in Marschall technique or a replacement for the anterior cruciate ligament using Hamstring ligaments and fixated them by using the transfix technique.Up till now, none of the patients showed any signs of ill-functioning growth plates.Though it may be necessary to point out that the patiens had not fully grown out by the time the subsequent check ups were held.The knees we revised were all stable and after relating our results to the IKDC-Score we found that our treatment reflected a good to very good result (Group A and B). Results of Marschall-Technique are very poore, results after boney refixations are good, result of our prospective study encourage us to recommend the surrogate of the anterior cruciate ligament by using the transfix technique in cases where the growth plates are still open. Therefore we can recommend reconstruction on transfix technique and refixation of boney ruptures.
