Die mediale Schenkelhalsfraktur Kontroversen in der Behandlung

Zusammenfassung

Die Behandlung der medialen Schenkelhalsfraktur bleibt bis in die heutige Zeit kontrovers. Ziel der vorliegenden Arbeit soll es sein, anhand der verfügbaren Literatur Richtlinien aufzuzeigen, womit schließlich die beste Therapie gewählt werden kann: konservativ vs. operativ, Osteosynthese vs. Prothetik, Zeitpunkt der Osteosynthese, offene vs. geschlossene Reposition, Implantatwahl bei der Osteosynthese, postoperative Nachbehandlung der Osteosynthese (belasten vs. entlasten), Pauwels-Osteotomie vs. Prothese bei der Schenkelhalspseudarthrose, Kopfprothese vs. Totalendoprothese, bipolare vs. monopolare Kopfprothese, welche Klassifikation soll angewendet werden. Weiter soll gezeigt werden, inwieweit die verschiedenen Standpunkte wissenschaftlich unterbaut sind und wo weitergehende Forschung notwendig ist.

Abstract

The treatment of the medial femoral neck fracture remains controversial until today. The goal of this paper is therefore, based on the literature, to show guidelines for optimal treatment: conservative treatment vs. operation, osteosynthesis vs. prosthesis, timing for osteosynthesis, open vs. closed reduction, choice of implant for osteosynthesis, postoperative treatment (weight bearing vs. non weight bearing), Pauwels-Osteotomy vs. prosthesis in cases op pseudarthrosis, femoral head prosthesis vs. total hip arthroplasty, bipolar vs. monopolar femoral head prosthesis, choice of classification. Further we want to point out which statements are evidence based and where we need further investigation.
