Zusammenfassung
Hintergrund: Die kathetergesteuerte endovaskuläre Therapie hat in der Behandlung arteriovenöser Malformationen des ZNS zunehmend an Bedeutung gewonnen. Anhand eigener Ergebnisse werden die Möglichkeiten und Grenzen dieser Therapieoption dargelegt. Methode: 19 Kinder und Jugendliche mit zerebralen arteriovenösen Malformationen [V. Galeni-Malformation (n=7), pialen arteriovenösen Malformationen (n=8) und AV-Fisteln der A. vertebralis (n=4)] wurden im Alter von 3 Monaten bis 17 Jahren mit der Mikrokathetertechnik über einen transarteriellen Zugang endovaskulär behandelt. Ergebnisse: Bei allen Patienten wurden eine deutliche Reduktion des Shunt-Volumens erzielt. Bei 7 Kindern gelang endovaskulär eine komplette Ausschaltung der Gefäßfehlbildung, bei 1 weiteren Jungen konnte nach mehrfacher endovaskulärer Vorbehandlung die Malformation neurochirurgisch entfernt werden. Die anderen Patienten erwarten mit einer Ausnahme eine Fortführung der interventionellen Therapie. Ein 2jähriges Kind erlitt im Anschluß an die erste Embolisationsbehandlung eine ausgeprägte Hirnblutung mit dauerhafter neurologischer Schädigung. Keiner der übrigen Patienten weist nach einer mittleren Nachbeobachtungszeit von 26 (7–95) Monaten persistierende therapieinduzierte neurologische Ausfallerscheinungen auf. Die als Säuglinge und Kleinkinder behandelten Patienten zeigen eine unauffällige statomotorische und psychomentale Entwicklung. Schlußfolgerung: Mit der superselektiven Embolisation arteriovenöser Malformationen des ZNS steht eine potente und mit vertretbarem Risiko durchzuführende Therapieoption zur Verfügung, die auch bei jungen Säuglingen erfolgreich eingesetzt werden kann.
Summary
Background: Endovascular management of cerebral arteriovenous malformations has gained increasing importance in recent years. Based on our own results the potentials and limitatons of this therapeutic option are stated. Methods: 19 children with cerebral arteriovenous malformations [vein of Galen malformation (n=7), pial arteriovenous malformations (n=8), AV-fistula of the vertebral arteries (n=4)] underwent superselective transarterial endovascular management of these lesion between 3 months and 17 years of age. Results: Complete angiographic exclusion was achieved in 7 patients. Endovascular pretreatment made subsequent neurosurgical exclusion of a pial arteriovenous malformation possible in one additional patient. The other children profited clinically by significant shunt reduction with improvement or disappearance of their presenting symptoms, all but one are under ongoing endovascular treatment. A 2-year-old girl suffered from massiv intracranial hemorrhage following her first embolization therapy with permanent neurologic deficit. Two transient but no permanent neurological complications occurred in the other patients now free of therapy induced neurological deficits after a medium follow-up of 26 (7–95) months. The children treated during infancy develop neurologically normal and without delay. Conclusion: Burdened with a justifiable risk superselective embolization of arterial feeders is a highly effective therapy in cerebral arteriovenous malformations and has been successfully applied even in young infants.
