Fluor vaginalis und rezidivierende perianale Fisteln durch eine präsakrale Dermoidzyste

Zusammenfassung

Bei einem 5jährigen Mädchen fand sich als Ursache eines therapierefraktären Fluor vaginalis und rezidivierender perianaler Fisteln eine präsakral gelegene Dermoidzyste. Eine Darstellung des Fistelsystems mit Kontrastmittel ermöglichte die genaue Klärung der anatomischen Situation. Dies war die Voraussetzung für eine problemlose Exstirpation. Präsakrale Tumoren sind selten und von ganz unterschiedlicher Pathologie. Gemeinsam ist ihnen ihre diskrete klinische Symptomatik. Eine frühzeitige Diagnosestellung ist nur möglich, wenn Hinweise, wie etwa rezidivierende perianale Fisteln, beachtet werden.

Summary

The case of a five-year-old girl with intractable fluor vaginalis and recurrent perianal fistulas is reported. A presacral dermoid cyst was shown to be the cause of these symptoms. Extensive radiological investigation revealed the cyst and the associated fistulas extending into the perianal region and the vagina. After verification of the anatomical situation, an uneventful surgical procedure was carried out.

A relatively infrequent and heterogeneous group of tumors may arise in the presacral space. Because of the vague nature of the symptoms, most of these lesions are found only by careful examination. Recurrent perianal fistulas should be a warning when routine surgery cannot resolve the problems.
