Zusammenfassung
Die meisten Anomalien des Spektrums der „congenital anomalies of the kidney and urinary tract“ (CAKUT) manifestieren sich bereits in der Pränatalperiode und können durch eine gezielte Ultraschalldiagnostik erkannt werden. Zum Ausschluss komplexerer Anomalien sollte immer eine detaillierte sonographische Beurteilung aller fetalen Organsysteme erfolgen.
Während einige Erkrankungen, wie die milde Hydronephrose, häufig transiente und daher lediglich kontrollbedürftige Befunde darstellen, sind Befunde aus dem Spektrum der „lower urinary tract obstruction“ (LUTO) häufig mit deutlich schwerwiegenderen Konsequenzen vergesellschaftet. Insbesondere Erkrankungen, die bereits im 2. Trimenon mit einer reduzierten Fruchtwassermenge assoziiert sind, weisen eine erhöhte pulmonale Morbidität und Mortalität auf. Pränatale Ultraschalluntersuchungen erlauben nur bedingt die Beurteilung der fetalen Nierenfunktion; jedoch können Fruchtwassermenge, kortikomedulläre Differenzierung sowie Echogenität des Nierenparenchyms als indirekte Parameter dienen. Die vorgeburtliche Beratung von Eltern sollte an ausgewiesenen Zentren für pränatale Diagnostik und Therapie unter Einbeziehung von multiprofessionellen Teams erfolgen.
Abstract
Most anomalies from the spectrum of congenital anomalies of the kidney and urinary tract (CAKUT) are already manifested in the prenatal period and can be detected by targeted ultrasonographic diagnostics. A detailed sonographic evaluation of all fetal organ systems should always be performed to rule out more complex anomalies.
While some diseases, such as mild hydronephrosis, are often transient and therefore only require monitoring, findings from the spectrum of lower urinary tract obstructions (LUTO) are often associated with much more severe consequences. In particular, conditions associated with reduced volume of amniotic fluid as early as the 2nd trimester, show an elevated pulmonary morbidity and mortality. Prenatal ultrasound examinations enable only limited assessment of fetal renal function; however, the volume of amniotic fluid, corticomedullary differentiation, and echogenicity of the renal parenchyma can serve as indirect parameters. Prenatal counselling of parents should be performed at designated centers for prenatal diagnostics and treatment involving multiprofessional teams.
