Zusammenfassung

Endoskopie und histologische Untersuchungen sind von entscheidender Bedeutung für die Diagnose von chronisch-entzündlichen Darmerkrankungen (CED). Auf der Basis der endoskopisch-makroskopischen und histologischen Befunde erfolgt die Zuordnung zur spezifischen Diagnose, also entweder M. Crohn, Colitis ulcerosa oder auch zur sog. unklassifizierten Kolitis. Außerdem werden Erkrankungsausdehnung und Schwere der Entzündung charakterisiert sowie die Befunde anhand der Paris-Klassifikation kategorisiert. Dazu sind regelhaft eine Ileokoloskopie, eine Ösophagogastroduodenoskopie und in einzelnen Fällen auch eine Videokapselendoskopie erforderlich. Sowohl bei der Diagnosestellung als auch im Verlauf der Erkrankung haben Endoskopie und histologische Untersuchungen direkte Bedeutung für die Therapie. Moderne Therapiestrategien streben die Mukosaheilung an.

Abstract

Endoscopy and histological investigations are of fundamental importance for the diagnosis of inflammatory bowel diseases (IBD). The specific diagnosis of either Crohn’s disease, ulcerative colitis or so-called unclassified colitis is made based on the results of the macroscopic findings during endoscopy and the results of the histological evaluation. In addition, disease extension and severity of inflammation are characterized and subsequently categorized according to the Paris classification. Ileocolonoscopy and esophagogastroduodenoscopy are regularly necessary and in isolated cases video capsule endoscopy is also needed. Endoscopy and histological investigations are relevant for the treatment and the diagnostics as well as during the course of the disease. Modern therapeutic strategies aim at the goal of mucosal healing.
