Fieber, Schüttelfrost und schwarze Finger

Zusammenfassung

Ein 18-Jähriger mit Fieberschüben, Schüttelfrost und wechselnden Arthralgien diverser Gelenke wurde unter der Verdachtsdiagnose septischer Embolien der Finger zu uns verlegt. Die Diagnose einer Polyarteriitis nodosa konnte gemäß den Kriterien der European League against Rheumatism (EULAR)/Paediatric Rheumatology European Society (PReS) gestellt werden. Der Verlauf dieser im Kindesalter sehr seltenen Erkrankung unter Therapie mit Iloprost, Cyclophosphamidstoßtherapie, Bosentan und Prednison wird präsentiert.

Abstract

We report on an adolescent with fever, chills and arthralgia who was referred to our hospital under suspicion of septic embolism of the fingers. The diagnosis polyarteriitis nodosa was made according to the criteria of the European League against Rheumatism (EULAR)/ Paediatric Rheumatology European Society (PReS). Polyarteriitis nodosa is a rare disorder in childhood and adolescence. We present the clinical course under treatment with iloprost, cyclophosphamide pulses, bosentan and prednisone showing clinical fotos and scanning laser doppler flowmetry of the fingers.
