Zusammenfassung
Nutzen und Risiken der intrathekalen Baclofentherapie (ITB-Therapie) als Palliativmaßnahme bei Kindern mit schwerster Behinderung wurden dokumentiert und bewertet, klinische Daten der Patienten, Therapieeffekte sowie Nebenwirkungen und Komplikationen wurden analysiert. Über einen Spinalkatheter mit Baclofen getestet wurden 51 Kinder mit schwerster Spastik oder Dystonie bei einem heterogenen Spektrum von Erkrankungen des zentralen Nervensystems (ZNS). Bei 37 Patienten mit positivem Testergebnis erfolgte die Implantation einer Baclofenpumpe. Bei 30 von ihnen wurden die angestrebten Therapieziele in vollem Umfang erreicht. Die Behandlung war nebenwirkungsarm, sodass Abbrüche nicht erforderlich wurden. Chirurgisch revisionsbedürftige Komplikationen betrafen 6 der 37 Patienten und waren überwiegend durch Funktionsstörungen des Pumpenkatheters verursacht. Die ITB-Therapie ist eine bedeutsame Maßnahme zur Verbesserung der Lebensqualität behinderter Kinder mit schweren spastischen und dystonen Bewegungsstörungen. Eine Abstimmung mit den Angehörigen über die Therapieziele und eine vorangehende Testung dienen der optimalen Patientenselektion für diese logistisch aufwändige und invasive Therapieoption.
Abstract
In the present study, benefits and risks of intrathecal baclofen treatment (ITB) as a palliative therapeutic procedure for children with severe disability were assessed and evaluated. Clinical data of patients, treatment effects, adverse events, and complications were analyzed. Fifty-one children with severe spasticity or dystonia due to a heterogeneous spectrum of disorders of the central nervous system were initially tested for baclofen efficacy using a spinal catheter. In 37 children with positive testing, a baclofen pump was implanted. In 30 out of 37 patients, targets of the therapeutic intervention were completely achieved. With ITB, only a few minor adverse events were reported, which did not lead to any interruption in treatment. In six of 37 patients, surgical revision of the baclofen pump system was necessary, mostly due to catheter dysfunction. ITB is an important palliative procedure to improve quality of life in severely disabled children and adolescents with spasticity and dystonia. Prerequisites for optimal patient selection for this laborious and invasive therapeutic option include baclofen testing prior to the implantation of the pump system and the presetting of individual therapeutic targets together with the patient’s caregivers.
