Zusammenfassung
Untersuchungen zur Lebensqualität (LQ) in der Kieferorthopädie (KFO) sind in Deutschland rar. Ziel der Studie war es daher, die Einflüsse der Behandlung auf die Lebensqualität zu untersuchen und ein valides Messinstrument zu entwickeln.
Die prospektive Studie wurde in einer großen kieferorthopädischen Praxis als „Cross-sectional-Erhebung“ angelegt. Dabei wurden 116 Kinder bzw. Eltern vor und 119 nach Abschluss der Behandlung befragt. Untersucht wurde mit einem Fragebogen zur LQ bei Kindern (KINDL) und einem Modul für KFO-Patienten. Zusätzlich wurden klinische und ökonomische Parameter erfasst.
Es zeigten sich signifikante Unterschiede zwischen der Eltern- und Kindereinschätzung in den Bereichen des körperlichen und psychischen Wohlbefindens. Erwartungen an „gehänselt werden“ vor und nach der Behandlung waren signifikant unterschiedlich. Kosmetisch-ästhetische Aspekte stellten die Hauptmotivation zur Behandlung dar. Das Zahnputzverhalten änderte sich signifikant hinsichtlich der Effektivität, jedoch nicht in der Frequenz.
Die vorliegende Untersuchung konnte die für Kinder wichtigen Faktoren für die Compliance während der Behandlung aufzeigen und den Stellenwert der Prophylaxemaßnahmen nachweisen. LQ-Erhebungen bieten so einen Ansatz zur Qualitätssicherung in der Praxis.
Abstract
Studies measuring quality of life (QoL) during orthodontic treatment are rare in Germany. Therefore, the aim of the present study was to measure the impact of orthodontic treatment on QoL and to develop a reliable instrument for measuring this.
The study was conducted in a large orthodontic practice and was designed as a prospective cross-sectional assessment. A total of 116 children or their parents were questioned before and 119 after the treatment. We used a validated questionnaire for children (KINDL) and a module for orthodontic patients. In addition, clinical parameters and economic outcomes were measured.
The results differed significantly between children and parents for ratings of physical and psychological wellbeing. Expectations of “being teased” about wearing braces were significantly different before and after treatment. Aesthetic aspects were the main motivation for having the treatment. Tooth-cleaning became significantly more effective, although the children did not brush their teeth more frequently.
This study has highlighted important factors in compliance in children during orthodontic treatment and the value of prophylactic measures. The assessment of QoL is thus relevant to quality assurance in the specialist practice.
