Zusammenfassung

Die perkutane endoskopische Gastrostomie wird bei Kindern zur Ernährung eingesetzt. Das Auftreten von Komplikationen nach Entfernung der PEG-Sonde ohne endoskopische Unterstützung ist nur selten beschrieben. Wir berichten von einem Patienten, bei welchem die PEG-Sonde ohne endoskopische Hilfe entfernt und die Halteplatte in situ belassen wurden. Diese verlegte die Ileozäkalklappe und führte zu einem Ileus mit Perforation und schwerer Peritonitis. Diese lebensbedrohliche Komplikation ließe sich durch ein endoskopisches Entfernen der PEG-Sonde vermeiden.

Abstract

Percutaneous endoscopic gastrostomy (PEG) is used in children for enteral feeding. Complications after removing the PEG tube without an endoscopic procedure are rarely described. We report on a male patient who developed bowel obstruction with perforation and severe peritonitis after removal of the PEG tube without endoscopic assistance; the “bumper” obstructed the ileocecal valve. Endoscopic removal of PEG tubes may avoid similar life-threatening complications.
