Zusammenfassung
Zystische Lungenfehlbildungen sind seltene Anomalien, die zunehmend häufiger in der pränatalen Sonographie festgestellt werden. Obgleich in den ersten Lebensmonaten Atemnotzustände und später auch pulmonale Infektionen auftreten können, bleiben doch die meisten Kinder zunächst symptomfrei. Es werden 2 Fälle mit zystischer Lungenfehlbildung beschrieben, die pränatal erfasst und nach der Geburt durch Thoraxnativaufnahmen, Thorax-HRCT und Sonographie mit Farbdoppler schließlich als bronchogene Zyste und intralobäre Lungensequestration klassifiziert wurden. Bei den völlig asymptomatischen Kindern erfolgte im Alter von 4 1/2 Jahren bzw. 2 1/2 Monaten eine Lungenlappenresektion, wobei sich die Verdachtsdiagnosen in der histopathologischen Untersuchung bestätigten. Beide Fälle wurden nach der Operation über einen Zeitraum von 7 Monaten bzw. 1 1/2 Jahren nachbeobachtet und haben bisher keine respiratorischen Komplikationen entwickelt. Eine frühzeitige Diagnose von zystischen Lungenfehlbildungen ist bereits durch die pränatale Sonographie in der Fetalperiode anzustreben. Eine operative Therapie sollte dann auch bei den asymptomatischen Kindern eher frühzeitig erfolgen, um das Auftreten von Atemnotzuständen oder potenziell lebensbedrohlichen Pneumonien zu verhindern.
Abstract
Congenital cystic lung lesions make up a group of rare anomalies, which are detected by antenatal ultrasonography with increasing frequency. While respiratory distress may develop in the early months of life and pulmonary infection in older children, most cases are initially asymptomatic. We report two cases of congenital cystic disease that were picked up by prenatal ultrasonography; in these cases postnatal chest radiograph, high-resolution chest CT and Doppler ultrasonography allowed the diagnoses of a bronchogenic cyst and pulmonary sequestration. Elective lobectomy was performed in both asymptomatic patients at the ages of 4 1/2 years and 2 1/2 months, and histolology confirmed the diagnosis. The postoperative course was uneventful in both cases, and after 7 months and 1 1/2 years of follow-up, respectively, both are doing well. Early recognition of congenital cystic lesions should be attempted antenatally by ultrasonography, and elective surgery should be performed at an early stage even when the young patients have no asymptoms, to avoid respiratory distress and potentially life-threatening pulmonary infection.
