Verlauf, Therapie und Komorbidität bei Hashimoto-Thyreoiditis im Kindesalter

Zusammenfassung

Hintergrund, Fragestellung

In einer multizentrischen Studie wurden Verlauf, Therapie und Komorbidität bei Hashimoto-Thyreoiditis im Kindes- und Jugendalter retrospektiv untersucht.

Methode und Patienten

Daten von 130 Patienten (108 weiblich, 22 männlich) mit einem mittleren Alter von 11,1±3,3 Jahren, die sich zwischen 1985 und 1999 vorstellten, wurden mit Hilfe eines standardisierten Fragebogens erfasst.

Ergebnisse

Jeweils 70, 55 und 13 Patienten hatten mindestens eine euthyreote, hypothyreote oder hyperthyreote Phase. Wurden euthyreote Patienten mit L-Thyroxin behandelt, zeigten sich sonographisch keine Reduktion des Schilddrüsenvolumens und kein Unterschied in der Veränderung des Schilddrüsenvolumens gegenüber nicht behandelten euthyreoten Patienten. Bei 20% der Patienten bestand eine nachweisbare Komorbidität, an erster Stelle Diabetes mellitus Typ 1, gefolgt von weiteren autoimmunologisch bedingten Endokrinopathien und chromosomalen Aberrationen.

Schlussfolgerungen

In der vorliegenden Studie zeigt die Behandlung der euthyreoten Hashimoto-Thyreoiditis mit L-Thyroxin keine Vorteile. In der Betreuung von Patienten mit Hashimoto-Thyreoiditis muss mit der Manifestation weiterer Autoimmunendokrinopathien gerechnet werden.

Abstract

Background

The present multicenter study was designed for a retrospective assessment of the clinical course, therapy and co-morbidity of autoimmune thyroiditis in childhood and adolescence.

Patients and methods

130 patients (108 female, 22 male) with a mean age of 11.1±3.3 years were evaluated on the basis of patient charts using standardized questionnaires.

Results

In the total group of 130 patients 70, 55, and 13 periods of euthyroidism, hypothyroidism and hyperthyroidism, respectively, were observed. Treatment of euthyroid autoimmune thyroiditis with L-thyroxine was of no benefit with regard to a reduction in thyroid gland volume as evaluated by ultrasound examination. 20% of patients suffered from substantial co-morbidity, among which diabetes mellitus type 1 was the commonest.

Conclusions

The present study suggests no advantage to the treatment of euthyroid autoimmune thyroiditis with L-thyroxine. Special attention should be paid to the early detection of co-morbidity associated with autoimmune thyroiditis of childhood and adolescence.
