Zusammenfassung
Hintergrund
Die Langzeit- und Hochdosistherapie mit Steroiden gilt als ein wesentlicher Risikofaktor der sekundären Osteoporose, einer typischen Komplikation im Krankheitsverlauf der zystischen Fibrose (CF). Die Option der konsekutiven Bisphosphonattherapie ist für das Kollektiv jugendlicher Mukoviszidosepatienten bisher wenig untersucht.
Fallbericht
Eine 18-jährige CF-Patientin erlitt nach langjähriger Hochdosissteroidtherapie eine atraumatische Wirbelkörperkompressionsfraktur. In der durchgeführten Osteodensitometrie zeigte sich eine deutlich Verminderung der Knochendichte. Durch die konsekutiv begonnene i. v. Therapie mit dem Bisphosphonatpräparat AREDIA® kann nach 1 1/2 Jahren bei guter Verträglichkeit der Behandlung eine signifikante Anhebung der Knochendichte erzielt werden.
Schlussfolgerung
Die erfolgreiche Behandlung der sekundären Osteoporose einer jugendlichen CF-Patientin mit einem i. v. Bisphosphonatpräparat sollte Anlass geben, auch in größeren Kollektiven jugendlicher Mukoviszidosepatienten, insbesondere unter Steroidtherapie, Messungen der Knochendichte zur Bestimmung einer therapierelevanten Osteopenie durchzuführen. Bei vorliegender Frakturgefährdung sollten eine gezielte Therapie sowie eine Kontrolle des Therapieerfolgs angestrebt werden, um in der Therapie der sekundären Osteoporose bei Kindern und Jugendlichen wertvolle Erfahrungen sammeln zu können.
Abstract
Background
Osteoporosis is a typical complication in long-term follow up in cystic fibrosis (CF). High dose cortison therapy plays a major role in its development. For the therapy of these patients with bisphosphonates, a standard treatment for women with postmenopausal osteoporosis studies and experience are missing.
Case report
An 18- year old patient with CF suffers from an atraumatic compression fracture of the lumbar spine based on generalized osteopenia after several years of treatment with high dose cortison. The osteodensitometry shows significantly reduced bone mineral density (BMD). We started an intravenous treatment with the bisphosphonate AREDIA®. A significant increase in BMD in osteodensitometry could be shown after one and a half year of treatment.
Conclusion
The successful treatment of osteoporosis in an 18-year old CF patient should be an impulse for larger studies in younger patients with measurement of BMD and consecutive bisphosphonate treatment if indicated. Therefore valuable experiences can be made in treatment of osteoporosis in children and young adults.
