Hematometrocolpos

A rare cause of acute obstructive urinary retention in 3 pubertal girls

Zusammenfassung

Kasuistik. Wir berichten über 3 Mädchen im Alter von 12–15 Jahren, die mit einer längeren Anamnese von Rückenschmerzen sowie akuten krampfartigen Unterbauchschmerzen zur Aufnahme kamen. Bei allen Mädchen konnte sonographisch eine leichte bis mäßiggradige Pyelonektasie mit erweiterten Ureteren nachgewiesen werden. Bei 1 Mädchen bestand zusätzlich ein akuter Harnverhalt. Bei allen Patientinnen fand sich ein ausgeprägter zystischer retrovesikaler Unterbauchtumor, der zur Obstruktion der ableitenden Harnwege geführt hatte. Dem zystischen Unterbauchtumor saß kappenartig der Uterus auf, dessen Lumen bei 2 Mädchen erweitert war. Die klinische Untersuchung erbrachte bei allen 3 Mädchen eine bläulich-livide, prallelastische, intralabiale Vorwölbung, die einer Hymenalatresie entsprach. Nach Resektion der Hymenalplatte bildeten sich bei allen Mädchen die Urinretention und die Pyelonektasie spontan zurück.

Abstract

Case reports. We report on 3 girls aged 12 to 15 years with a history of recurrent backache as well as acute lower abdominal pain. In all three adolescents sonography revealed bilateral pyelectasis as well as dilated ureters. One girl additionally had acute urinary retention. In all 3 girls a huge retrovesical cystic tumor could be shown, which had caused obstruction of the urinary tract. The uterus was located at top of the cystic lower abdominal mass. In two patients the cavum of the uterus was dilated. Clinical examination revealed an intralabial protruding mass caused by an imperforate hymen. After resection of the hymenal plate prompt recovery of urinary retention and pyelonectasy occurred.
