Zusammenfassung

Wir berichten über einen 72-jährigen Patienten, der uns mit rezidivierendem Fieber, Nachtschweiß und Gewichtsverlust bei initial gesicherter mediastinaler Lymphadenopathie am Zentrum für Seltene Erkrankungen vorgestellt wurde. Mit einer Diagnostik anhand von Lymphknotenmaterial konnte keine Diagnosesicherung erreicht werden. Nebenbefundlich bestand ein myelodysplastisches Syndrom. Über eine Knochenmarkpunktion und gezielte molekulargenetische Diagnostik ließ sich ein VEXAS-Syndrom („vacuoles, E1 enzyme, X‑linked, autoinflammatory, somatic“) diagnostizieren.

Abstract

The case of a 72-year-old male patient who presented to our centre for rare diseases with recurrent fever, night sweats and weight loss with initially confirmed mediastinal lymphadenopathy is reported. Investigation of lymph node material was unrevealing. As an additional finding, the patient had a myelodysplastic syndrome. VEXAS syndrome (vacuoles, E1 enzyme, X‑linked, autoinflammatory, somatic) could be confirmed on the basis of a bone marrow biopsy and genetic testing.
