Zusammenfassung
Wir berichten über eine 45-jährige Patientin, die sich mit ausgeprägter muskulärer Schwäche vorstellte. Aus der Befundkonstellation einer Hypokaliämie, Hyperkaliurie, metabolischen Azidose und eines basischen Urins ergab sich die Diagnose einer renalen tubulären Azidose Typ I. Nach Kaliumsubstitution und Verabreichung einer alkalisierenden Substanz war die Beschwerdesymptomatik der Patientin rückläufig. Bei der Patientin wurde gezielt nach häufig assoziierten Autoimmunerkrankungen gefahndet: Eine primär biliäre Zirrhose wurde diagnostiziert und die Therapie mit Ursodeoxycholsäure eingeleitet.
Abstract
We report a 45-year-old female patient with muscle weakness. We diagnosed renal tubular acidosis type I by laboratory findings of hypopotassemia, hypopotassuria, metabolic acidosis and basic urine. The muscle weakness improved rapidly by substitution of potassium and an alcalescent substance. Searching for associated autoimmune diseases we diagnosed primary biliary cirrhosis and initiated a therapy with ursodeoxycholic acid.
