Zusammenfassung
Bei dem vorgestellten Patienten wurde eine vermutlich kongenitale Hypophyseninsuffizienz erst in einem Alter von 38 Jahren diagnostiziert. Trotz des partiellen Ausfalls sämtlicher Hormonachsen des Hypophysenvorderlappens gab es bisher keine Krankenhausaufenthalte aufgrund akuter, durch den Hormonmangel verursachter Probleme oder infolge einer akuten Erkrankung, welche durch den Hormonmangel aggraviert wurde. Durch die bestehenden Hormonausfälle ist der Patient jedoch dysmorph und in seiner intellektuellen und physischen Leistungsfähigkeit deutlich eingeschränkt. Die Einleitung einer Hormonersatztherapie mit Hydrokortison und Schilddrüsenhormonen ist lebenswichtig, eine Substitution mit Geschlechtshormonen kann jedoch ethische Fragen aufwerfen.
Abstract
We present the case of a patient with suspected congenital hypopituitarism first diagnosed at the age of 38 years. Despite partial insufficiency of all pituitary-regulated hormonal axes, the patient never suffered from severe health problems. However, the patient was disfigured, and his intellectual and physical capacities were clearly impaired. The initiation of a hormone replacement therapy with hydrocortisone and thyroid hormones is essential in such a patient, but the substitution of sex hormones can create ethical problems.
