Zusammenfassung
Ein 73-jähriger Patient wurde mit Hämatemesis und Kreislaufkollaps eingeliefert. Laborchemisch fielen massiv erhöhte Infektzeichen und eine transfusionspflichtige Anämie auf. Zwei Monate vorher war der Patient wegen einer Sepsis mit Staphylococcus aureus antibiotisch behandelt worden. Die damals durchgeführten bildgebenden Verfahren hatten lediglich eine thorakale Arteriosklerose ergeben. Die Notfallendoskopie zeigte nun eine derb wirkende Erhabenheit mit zentralem Mukosadefekt im distalen Ösophagus als Blutungsquelle. Der Patient verstarb etwa 10 h nach der Aufnahme an einer massiven gastrointestinalen Blutung. Bei der Obduktion fand sich überraschend ein rupturiertes thorakales Aortenaneurysma mit Perforation in den distalen Ösophagus bei hochgradiger allgemeiner Arteriosklerose. Die rasche Bildung eines Aneurysmas mit Ruptur nach einer vorhergegangenen bakteriellen Infektion ist typisch für ein sog. mykotisches, d. h. infektiöses Aneurysma.
Abstract
A 73-year-old man was admitted to the hospital due to severe hematemesis and collapse, severe anemia and inflammation. Two months ago, the patient had been treated with antibiotics due to septicemia with staphylococcus aureus. At that time CT scan had shown only thoracic arteriosclerosis. The subsequent high urgency upper endoscopy identified a circular mucosal defect in distal esophagus as bleeding origin. The patient died 10 hours after admission. Performing autopsy, a fistula between the thoracic aortic aneurysm and the distal esophagus was found in the background of severe arteriosclerosis. The rapid onset of an aneurysm with rupture after a bacterial infection is typical for a mycotic aneurysm.
