Zusammenfassung
Das von Willebrand-Syndrom ist eine hereditäre hämorrhagische Diathese mit einer Prävalenz von etwa 1% in der Bevölkerung. Der Erkrankung liegen quantitative oder qualitative Defekte des von Willebrand-Faktors (VWF) zugrunde, einem hochmolekularen, multimeren Glykoprotein. Bei der Hämostase hat er im Wesentlichen 2 Funktionen: Zum einen ist er als Adhäsionsprotein zuständig für die Formation des Plättchenthrombus, zum anderen bindet er nicht-kovalent an den Gerinnungsfaktor VIII und schützt ihn dadurch vor vorzeitigem Abbau und Inaktivierung. Nach pathophysiologischen Gesichtspunkten ist das von Willebrand-Syndrom in 3 Kategorien eingeteilt: partiell quantitative Defekte des VWF beim Typ 1, qualitative Defekte beim Typ 2 und völliges Fehlen des VWF beim Typ 3. Klinisches Hauptmerkmal der Erkrankung ist die Neigung zu Schleimhautblutungen. Sofern der plasmatische Gerinnungsfaktor VIII deutlich erniedrigt ist, kann es auch zu Gelenkblutungen und Muskeleinblutungen kommen. Eine von 2 Säulen in der Therapie des von Willebrand-Syndroms ist die Gabe von Desmopressin, das die körpereigene Sekretion des VWF und von Faktor VIII steigert. Zweite Therapiesäule ist die exogene Zufuhr in Form von Plasmakonzentraten. Daneben können adjuvante Therapiemaßnahmen erwogen werden.
Abstract
Von Willebrand’s disease is an inherited bleeding disorder with a prevalence as high as 1% in the general population. The disease is caused by the quantitative deficiency or dysfunction of von Willebrand factor (VWF), a large multimeric glycoprotein. VWF has two main functions in hemostasis: it is essential for platelet-plug formation as an adhesion protein and it forms a non-covalent complex with coagulation factor VIII in plasma, thereby protecting it from inactivation and clearance. Inherited Von Willebrand’s disease has been subdivided into 3 categories that reflect pathophysiology: partial quantitative deficiency of VWF (Type 1), qualitative deficiency (Type 2) and total deficiency (Type 3). The major clinical hallmark in Von Willebrand’s disease is an increased tendency to mucocutaneous bleeding. Increased bleeding may also occur in sites such as muscles and joints when the level of factor VIII is particularly low. The mainstays of therapy are desmopressin, which induces secretion of autologous factor VIII and VWF into plasma, and plasma concentrates, which supply allogenic forms of these moieties. Other forms of treatment can be considered as adjunctive to these.
