Zusammenfassung
Wir berichten über einen 43-jährigen Patienten, der aufgrund wiederkehrender Bolusobstruktionen aufgenommen wurde. Schon seit der Kindheit beklagte er regelmäßige Dysphagien, Medikamente konnten nicht richtig geschluckt werden. Weiterhin beklagte er häufiges Sodbrennen. Die letzte Bolusobstruktion ereignete sich wenige Wochen zuvor. Nach der Entfernung des Bolus über ein starres Endoskop fand sich dort eine hochgradige, auch für ein Kindergastroskop nicht passierbare Stenose im proximalen Ösophagus. Die histopathologische Beurteilung bestätigte den endoskopischen Verdacht auf eine eosinophile Ösophagitis. Als Therapie der eosinophilen Gastritis kommt zunächst eine lokale Kortikoidtherapie in Frage. Dabei sollte eine Therapie über mindestens 6–9 Monate erfolgen. Bei Patienten mit typischer Ringbildung oder Stenosierung kann eine Bougierungsbehandlung erforderlich werden. Unser Patient war unter der eingeleiteten oralen Fluticasontherapie in der Kontrolle beschwerdefrei. Nach Absetzen der Medikation kam es jedoch zu einer Befundverschlechterung, und es wurde eine erneute medikamentöse Behandlung eingeleitet.
Abstract
We report a case of a 43 year old male patient, who was admitted with recurring esophageal bolus impactions. Since his childhood he has been complaining about dysphagia and was unable to swallow medication. He also complained about heartburn. The last esophageal bolus impaction was some weeks ago. After elimination of the bolus impaction with a rigid endoscope we found a high grade stenosis in the proximal esophagus that could not even be passed with a childrens’ endoscope. An initial treatment of eosinophile esophagitis would be the therapy with a local corticoid for 6–9 month. In patients with typical rings or stenosis a dilation therapy might be necessary.
