Zusammenfassung
Die traumatische Pfortaderthrombose ist eine seltene Ursache der nichtzirrhotischen portalen Hypertension. Der vorliegende Fallbericht beschreibt einen 19-jährigen Patienten, der mit Ösophagusvarizenblutung und Splenomegalie aufgenommen wurde. Bei histologisch unauffälligem Leberparenchym wurde die Diagnose einer traumatischen Pfortaderthrombose als Ausschlussdiagnose bei regelmäßigem Kampfsport (Kickboxen) gestellt. Daraufhin erfolgte eine endoskopische Varizenligatur und aufgrund der kavernösen Pfortadertransformation und des Hypersplenismus die Anlage eines splenorenalen (Warren-) Shunts. Hierunter wurde der Patient beschwerdefrei.
Abstract
Traumatic portal vein thrombosis is a rare cause of nonmalignant, noncirrhotic portal hypertension. We report a case of a 19-year old patient, who presented with variceal bleeding and splenomegaly. Diagnosis was based on the history of kickboxing and an otherwise negative etiological investigation. The patient underwent endoscopic therapy and portosystemic shunt operation (Warren-shunt) due to cavernous transformation and severe hypersplenism. Thereafter the patient remained asymptomatic.
