Zusammenfassung
Erfreulicherweise hat die durchschnittliche Lebenserwartung von Patienten mit Mukoviszidose (zystischer Fibrose, CF) in den letzten 30 Jahren deutlich zugenommen. Im deutschen CF-Register sind knapp die Hälfte der Patienten älter als 18 Jahre. Die assoziierten Symptome und Komplikationen sind dabei im Wesentlichen in allen Altersgruppen gleich, die Häufigkeit von pneumologischen und nicht pneumologischen Problemen nimmt aber mit dem Alter eher zu. Zu den Komplikationen der Mukoviszidose gehören u. a. respiratorische Globalinsuffizienz, pulmonale Hypertonie, Diabetes mellitus und Osteoporose. Zusätzlich kommen neue, für das Erwachsenenalter typische internistische Komplikationen hinzu, wie z. B. arterielle Hypertonie, Hyperlipidämien oder kardiovaskuläre Erkrankungen. Ferner treten Beziehungs- und Familienkrisen oder der Verlust des Arbeitsplatzes hinzu, und familienplanerische Maßnahmen wie In-vitro-Fertilisation betreffen ausschließlich Erwachsene. Deshalb sollten erwachsene CF-Patienten in einem auf Erwachsene spezialisierten Zentrum behandelt werden. Aktuell wird jedoch nur etwa ein Drittel der gut 4000 erwachsenen Patienten in Deutschland in reinen Erwachsenenambulanzen betreut, ein Großteil wird in altersunabhängigen Zentren sowie in vielen kleinen Ambulanzen betreut. In diesem Artikel werden die aktuell etablierten Transitionsmodelle sowie mögliche Hindernisse bei ihrer Umsetzung erläutert.
Abstract
During the last 30 years, life expectancy in patients with cystic fibrosis has significantly improved. In Germany, almost half of the 8500 patients are 18 years or older. Older patients have increased rates of cystic fibrosis typical complications, In addition the characteristic complications of adulthood, including arterial hypertension, hyperlipidemia, and cardiovascular diseases, occur. Also crisis of marriage or loss of work place, as well as family planning measures including in-vitro-fertilization are problems merely of the adult cystic fibrosis patient. Therefore adult patients should be treated in a centre specialized on adults. At the moment, in Germany only one third of all adult patients are followed up in an adult center, many patients are treated in age-independent centers, and also a significant number is treated in small clinics. In this article models for transition currently established in Germany are described and occurring problems with their implementation are discussed
