Zusammenfassung
Infektionen sind die häufigsten Komplikationen bei Patienten mit hämatologischen und onkologischen Erkrankungen unter Chemotherapie oder nach Stammzelltransplantation. Bei steigender Inzidenz maligner Erkrankungen und zunehmendem Einsatz von Chemotherapie auch in der palliativen Krankheitssituation wird auch der ambulant tätige Internist vermehrt mit dieser Komplikation konfrontiert werden. Das Spektrum der verantwortlichen Infektionserreger ist je nach Ausmaß der Schädigung der Granulopoese, der T-Zellen oder des Antikörpermangels unterschiedlich, sodass die Regeln zur Prävention, Diagnostik und Therapie für unterschiedliche Patientencharakteristika spezifiziert werden müssen. Unter Beachtung dieser Prämissen sind die Erfolgsraten antimikrobieller Prophylaxe und Therapie heute sehr hoch. So konnte mittlerweile die Intensität der Infektionstherapie für einige Patientengruppen reduziert werden. Präventionsmaßnahmen einschließlich Schutzimpfungen sollten nicht nur im stationären, sondern auch im ambulanten Bereich bekannt sein und beachtet werden.
Abstract
Infections are the predominant complications in patients with hematological and oncological diseases undergoing chemotherapy or stem cell transplantation. The spectrum of causative pathogens depends on the suppression of granulopoiesis, T-cell immunity or humoral deficiency, so that recommendations for prevention, diagnostic approach and antimicrobial treatment should be specified for each cohort of patients. Following these rules, outcomes of prophylaxis and treatment today are favorable, although for some patient populations, the intensity of antimicrobial intervention has been reduced. Recommendations for prevention, including vaccination, should be known and followed for hospitalized as well as for out-patients.
