Zusammenfassung
Das A.-mesenterica-superior-Kompressionssyndrom (Wilkie-Syndrom) ist eine seltene Erkrankung, bei der die Pars horizontalis des Duodenums von der ventral verlaufenden A. mesenterica superior und der dorsal liegenden Aorta abdominalis komprimiert wird. Die Symptomatik besteht in einem frühen Sättigungsgefühl, Übelkeit, in wiederkehrendem Erbrechen, postprandialem Abdominalschmerz und ungewolltem Gewichtsverlust. Die konservative Therapie mit intravenöser Hyperalimentation hat zum Ziel, durch Anreicherung des retromesenterialen Fettgewebes den Winkel zwischen A. mesenterica superior und Aorta aufzuweiten, um so wieder eine Duodenalpassage herzustellen. Reicht dies nicht aus, empfiehlt sich eine operative Therapie, die bisher in einer intestinalen Umgehungsoperation besteht. Wir berichten über ein Behandlungskonzept, bei dem durch die Transposition der A. mesenterica superior in die infrarenale Aorta das mechanische Problem kausal gelöst wird und durch eine wiederhergestellte physiologische Dünndarmpassage die Beschwerden des Patienten aufgehoben werden.
Abstract
The superior mesenteric artery syndrome – also known as Wilkie’s syndrome or as arteriomesenteric obstruction of the duodenum – is a rare condition of upper intestinal obstruction in which the third part of the duodenum is compressed by the overlying, narrow-angled superior mesenteric artery against the posterior structures. It is characterized by early satiety, recurrent vomiting, abdominal distention, weight loss and postprandial distress. When nonsurgical management is not possible or the problem is refractory, surgical intervention is necessary. Usually a reconstruction of the intestinal passage is performed. We report the first case of successful transposition of the superior mesenteric artery into the infrarenal aorta in the therapy of Wilkie’s syndrome.
