Zusammenfassung
Die Ösophagogastroduodenoskopie (ÖGD) hat die Röntgendiagnostik als Standardmethode zur Untersuchung des oberen Gastrointestinaltrakts abgelöst. Zusätzlich bietet die „Gastroskopie“ eine Reihe von minimal-invasiven Therapieoptionen. Im vorliegenden Beitrag werden die notwendigen medizinischen, technischen und personellen Voraussetzungen für die ÖGD dargestellt. So ist stets die Indikation dieser invasiven Maßnahme zu überprüfen, Kontraindikationen sind auszuschließen. Der Ablauf der ÖGD sollte in standardisierter Weise erfolgen und dokumentiert werden. Dabei sind Auffälligkeiten der Peristaltik zu beachten, Veränderungen der Schleimhaut nach Größe und Oberflächenbeschaffenheit zu beschreiben. Die endoskopischen Techniken der Biopsie, der Chromoendoskopie und der PEG-Anlage sollten in der Routineendoskopie jederzeit einsetzbar sein.
Abstract
Esophagogastroduodenoscopy (EGD) has replaced X-ray diagnosis as the standard method for assessment of the upper gastrointestinal tract. It also offers an array of minimally invasive treatment options. This contribution presents the requisite medical, technical, and human resources needed for EGD. The indication for this invasive procedure should always be carefully reviewed and contraindications excluded. EGD should be performed according to standardized procedures and well documented by noting distinctive features of peristalsis and describing alterations of mucosal size and surface texture. The endoscopic techniques of biopsy, chromoendoscopy, and percutaneous endoscopic gastrostomy should always be available in routine endoscopy.
