Zusammenfassung
Hintergrund und Fragestellung: Fehlbelegungsprüfungen von Seiten der Versicherungsträger gewinnen in Anbetracht begrenzter ökonomischer Ressourcen zunehmend an Bedeutung. Als Prüfinstrument bietet sich dazu das Appropriateness Evaluation Protocol (AEP) an. Es wurde 1981 in den USA für die Chirurgie und Innere Medizin entwickelt und bewertet anhand eines Kriterienkataloges die Angemessenheit stationärer Aufnahmen und Behandlungstage.
Patienten/Methodik: Mit der vorliegenden Arbeit soll die Praktikabilität des AEP für die Hals-, Nasen-, Ohrenheilkunde überprüft werden. Dazu wurden am gleichen Wochentag in vier aufeinanderfolgenden Wochen alle Akten der stationären Patienten der HNO-Klinik gesichtet und AEP-Bewertungen vorgenommen.
Ergebnisse: Insgesamt wurden unabhängig von drei Prüfern jeweils 196 Behandlungstage überprüft. Die gemittelte Fehlbelegungsrate nach AEP lag bei 41,5%. Bewertete man den präoperativen Tag vor Elektiveingriffen als angemessen, betrug die Fehlbelegungsrate 23,1%.
Schlussfolgerungen: Die kritische Gegenüberstellung dieses Ergebnisses mit den aktuellen Gründen für die stationäre Aufnahme bzw. Behandlung ergab, dass das AEP in der vorliegenden deutschen Version wenig als Prüfinstrument für Fehlbelegung in der Hals-, Nasen-, Ohrenheilkunde geeignet ist. Aus den Erfahrungen mit der Anwendung des AEP wird ein HNO-adaptiertes AEP (AEP-ENT) vorgeschlagen.
Abstract
Background and objective: Determination of inappropriate hospital use is of increasing importance due to limited resources in health care. The Appropriateness Evaluation Protocol (AEP) serves as an instrument to identify this inappropriate hospital use. It was designed in the USA in 1981 for use in Internal Medicine and General Surgery and consists of criteria items to assess the appropriateness of hospital admissions and days of care.
Patients/Methods: The present study aims to examine the practicability of the AEP in Otorhinolaryngology. The charts of all patients hospitalized in the ENT Department at that particular day were surveyed on 4 consecutive Wednesdays. Three reviewers each reviewed 196 charts.
Results: The overall level of inappropriate use was 41.5%. Presuming that the day preoperative to elective surgery was appropriate, the level was 23.1%.
Conclusions: The critical comparison between this result and the actual reasons for hospital admissions and days of care showed, that the present German version of the AEP is less suitable for use in Otorhinolaryngology. Based on the experiences with the AEP an AEP adjusted to Otorhinolaryngology (AEP-ENT) is proposed.
