Erstisolierung von Bilophila wadsworthia bei Otitis externa

Zusammenfassung

Bilophila wadsworthia ist ein 1989 erstmals beschriebenes gramnegatives, obligat anaerobes Stäbchenbakterium. Der natürliche Standort von Bilophila wadsworthia scheint in erster Linie der humane Intestinaltrakt zu sein, wo die Spezies bei etwa 60% der untersuchten Personen nachweisbar war. Klinische Isolate von Bilophila wadsworthia stammen überwiegend aus intraabdominalen Untersuchungsmaterialien. Als Bestandsteil mischinfizierter Prozesse wurde Bilophila wadsworthia auch bei extraabdominalen Infektionen bereits mehrfach isoliert, jedoch sind keine Nachweise im Zusammenhang mit otologischen Erkrankungen beschrieben worden. Wir stellen den Fall einer Patientin mit unmittelbar postoperativer Otitis externa nach Stapesplastik vor, bei der die Anaerobier Bacteroides fragilis und Bilophila wadsworthia als wahrscheinliche Erreger isoliert wurden.

Summary

Bilophila wadsworthia is an obligately anaerobic, gram-negative rod, which was first described in 1989. The natural habitat of this species seems to be the lower intestinal tract where it was recovered from about 60% of the persons investigated. Bilophila wadsworthia is mainly associated with intra-abdominal infections, but the species was also isolated as part of aerobic-anaerobic mixed infections at various extra-abdominal sites. Otological infections due to Bilophila wadsworthia have as yet not been described. In the present paper, we report a case of a patient with postoperative otitis externa following stapedectomy, probably caused by the anaerobes Becteroides fragilis and Bilophila wadsworthia.
