Zusammenfassung
Das Leiden am Tinnitus wird in seiner Entstehung und seiner Verarbeitung oft mit Stress in Verbindung gebracht. Dazu existieren vielfältige Veröffentlichungen, die mit unterschiedlichen Konzepten und unterschiedlichem Testinventar mögliche Zusammenhänge zwischen Stressempfinden und Leiden am Tinnitus untersucht haben. Hier wird die Entwicklung des Stressbegriffs von Selye über das transaktionale Stressmodell von Lazarus bis zur Übertragung auf das Leiden am Tinnitus aufgezeigt. Kritisch aufgearbeitet wird die Literatur, die mit teilweise sehr unterschiedlichen Vorstellungen den Einfluss von Stress beim Leiden am Tinnitus untersucht hat. Dabei ist es von Bedeutung, Stressauslöser und Stressreaktionen zu unterscheiden. So wird etwa nahegelegt, dass psychosozialer Stress die gleiche Wahrscheinlichkeit hat, zu einem Tinnitus beizutragen, wie Lärm am Arbeitsplatz. Auffällig in den bisherigen Untersuchungen ist, dass sich dennoch „Stress“ als Einflussgröße nicht eindeutig mit mehr oder weniger geeigneten psychometrischen Testverfahren verifizieren lassen konnte oder sich keine signifikanten Unterschiede zu – sehr unterschiedlichen Vergleichsgruppen – zeigten. Zum Abschluss wird eine mögliche therapeutische Herangehensweise zur Stressbewältigung skizziert.
Abstract
The development and processing of tinnitus is often associated with stress. There are many publications on this subject that have investigated possible connections between stress perception and tinnitus symptoms using different concepts and different test inventories. In this review, we present the development of Selye’s concept of stress using the transactional stress model of Lazarus and its transfer to patients suffering from tinnitus. The literature evaluating the influence of stress on tinnitus symptoms with partly very different concepts is critically reviewed. For example, it is suggested that psychosocial stress has the same likelihood of contributing to tinnitus as noise in the workplace. However, what is striking in previous studies is that “stress” as an influencing variable could not be clearly verified with suitable psychometric test procedures or that no significant differences—to very different comparison groups—could be shown. Finally, a possible therapeutic approach to stress management is outlined.
