Zusammenfassung
Malformationen des Ohrs stellen eine der häufigsten Fehlbildungen im Kindesalter dar. Diese unterteilen sich in Dysplasien der Ohrmuschel, Fehlanlagen des äußeren Gehörgangs und Fehlbildungen des Mittel- und Innenohrs. Letztere treten glücklicherweise relativ selten auf. Am häufigsten treten klinisch isolierte geringgradige Dysplasien der Ohrmuschel (Otabtosis, „cup ear“) in Erscheinung. Höhergradige Fehlbildungen der Ohrmuschel (Mikrotie II–III°) sind oft syndromgebunden und/oder mit Fehlbildungen des Gehörgangs und Mittelohrs assoziiert (z. B. Atresia auris congenita mit höhergradiger Mikrotie, Atresie des Gehörgangs und Dysplasie des Mittelohrs). Sowohl die funktionelle Bedeutung bezüglich des Hörens und des Spracherwerbs als auch ästhetische Aspekte betonen die Bedeutung der chirurgischen Möglichkeiten. Da die Wahl des altersentsprechenden optimalen Zeitpunkts einer Rekonstruktion von immenser Wichtigkeit ist, gibt der vorliegende Artikel einen Überblick sowohl über die Möglichkeiten der ästhetischen Rekonstruktion der Ohrmuschel als auch über die damit oft verknüpfte funktionelle Wiederherstellung des Gehörs.
Abstract
Malformations of the ear are the most common dysplasias in childhood. Ear malformations are classified into dysplasias of the auricle, of the ear canal, and of the middle and inner ear. The latter types are fortunately relatively rare. Most common is a low-grade dysplasia of the auricle (otapostasis, cup ear). Higher-grade dysplasia of the auricle (microtia II–III) is often associated with syndromes and/or malformations of the ear canal and the middle ear (e.g., congenital aural atresia with high-grade microtia and dysplasia of the middle ear). Functional aspects of hearing and acquisition of language as well as aesthetic aspects emphasize the importance of surgical therapy. Due to the importance of correctly scheduling surgical treatment in early childhood, this article gives an overview of the possibilities for aesthetic reconstruction of the auricle and the frequently associated functional rehabilitation of hearing.
