Zusammenfassung
Hintergrund
Obwohl eine Funktionsstörung der Eustachi-Röhre zu deutlichen Einschränkungen der Lebensqualität und der Funktion des Mittelohrs führen kann, gibt es bisher keinen Goldstandard zur Diagnostik von Dysfunktionen der Tuba auditiva.
Material und Methoden
McCoul et al. veröffentlichten 2012 den „Eustachian Tube Dysfunction Questionnaire“ (ETDQ-7) als Instrument zur Evaluation einer Tubenfunktionsstörung. McCoul wies die Reliabilität mit r = 0,78 nach Spearman und Cronbach-α ≥ 0,711 sowie die Validität des ETDQ-7 mit p < 0,001 nach. Der Grenzwert zur Diagnose einer Tubenfunktionsstörung wurde mit ≥ 14,5 bei 100 % Sensitivität und 100 % Spezifität angegeben. Wir übersetzten diesen Fragebogen ins Deutsche und testeten ihn an 100 Gesunden und 43 Patienten mit chronisch-obstruktiver Tubenfunktionsstörung.
Ergebnisse
Der mittlere Gesamtpunktewert des ETDQ-7 lag bei den Gesunden bei 8,67 und bei den Patienten mit chronisch-obstruktiver Tubenfunktionsstörung bei 24,7. Es ergab sich in unserer Analyse eine Sensitivität des ETDQ-7 von 90,7 und eine Spezifität von 95. Der positive prädiktive Wert lag bei 88,6 und der negative prädiktive Wert bei 96. Die Fläche unter der Kurve lag in unserer ROC-Analyse bei 98,8 % (p < 0,0001).
Schlussfolgerung
Wir empfehlen den ETDQ-7 zur Unterstützung der gezielten Anamnese bei Verdacht auf chronisch-obstruktive Tubenfunktionsstörung und als zusätzliches Instrument zur Verlaufskontolle nach operativer Therapie.
Abstract
Background
Eustachian tube dysfunction can cause middle ear diseases and impair quality of life. Sufficient diagnostic tools for chronic obstructive eustachian tube dysfunction are lacking and not reliable enough.
Patients and methods
The Eustachian Tube Dysfunction Questionnaire (ETDQ-7) was published by McCoul et al. in 2012 as an instrument to evaluate eustachian tube function. They demonstrated the reliability und validity of their questionnaire. The cut-off point for the diagnosis of eustachian tube dysfunction is ≥ 14.5 at 100 % sensitivity and 100 % specificity. We translated the ETDQ-7 into German and used it on 100 healthy subjects and 43 patients with chronic obstructive eustachian tube dysfunction.
Results
The results in the English questionnaire were confirmed by our examinations with the German version of the questionnaire. The mean ETDQ-7 score was 8.67 in the healthy subjects and 24.7 in the patients with chronic obstructive eustachian tube dysfunction. The area under the curve in ROC analysis was 98.8 % (p < 0.0001).
Conclusion
We recommend the ETDQ-7 as an addition to patient history in the examination of eustachian tube dysfunction. It may as well be valuable in follow-up examinations to monitor treatment success.
