Zusammenfassung
Bei Störungen der Rezeptorfunktion der Bogengänge besteht ein pathologischer angulärer vestibulookulärer Reflex (aVOR). Die daraus resultierende gestörte Blickstabilisierung wird als „Schwindel“ interpretiert und ist infolge einer Rezeptorfunktionsstörung aller Bogengänge möglich. Deren Objektivierung erfolgt mittels Video-Kopfimpulstest (KIT). Dabei zeigen sich verdeckte (Catchup-covert-) oder sichtbare (Catchup-overt-) Rückstellsakkaden. Die meisten peripher-vestibulären Störungen lassen sich konservativ behandeln. Chirurgische Therapieoptionen gibt es z. B. beim benignen paroxysmalen Lagerungsschwindel (meist hinterer Bogengang) und beim Syndrom der Dehiszenz der oberen Bogengänge. Vestibuläres Training fördert die zentrale Neueinstellung des VOR. Vor allem beidseitige Störungen der Rezeptorfunktion mit Beteiligung der Bogengänge beeinflussen Navigation, Raumorientierung, zeitliche Wahrnehmung und kognitive Prozesse. Derzeit werden Gleichgewichtsprothesen entwickelt, die künftig als „Ersatz“ für Rezeptoren implantiert werden könnten.
Abstract
Perturbation of semicircular canal function may result in a pathological angular vestibulo-ocular reflex (aVOR). The resulting impairment in gaze stabilization is perceived as “vertigo” or “dizziness” and may occur following receptor function impairment of all three semicircular canals. The head impulse test reveals hidden (covert-catchup) or visible (overt-catchup) saccades in disturbances of semicircular function. Most peripheral vestibular disorders can be treated conservatively. There are surgical treatment options for some diseases, such as intractable benign paroxysmal positional vertigo and superior semicircular canal dehiscence. Vestibular training promotes central reorganization of the VOR. Impaired semicircular receptor function, in particular bilateral vestibulopathy, may affect spatial orientation and cognitive processes. Balance prostheses could serve as a replacement for receptors in the future.
