Zusammenfassung
Die Versorgung mit einem Cochleaimplantat (CI) ist auch bei Patienten mit schweren Grunderkrankungen die Therapie der Wahl bei an Taubheit grenzender Schwerhörigkeit. Wir berichten über den Fall einer CI-Dysfunktion nach kardialer Defibrillation mit anschließender Verschlechterung des Sprachverständnisses, Verstärkung eines Tinnitus sowie neuen Kopfschmerzen. Ein Integrity-Test zeigte verschiedene Elektrodenanomalien, ohne dass ein direkter Zusammenhang zur Kardioversion gezeigt werden konnte. Wir führten die Ex- und Reimplantation des CI durch. Im Rehabilitationsverlauf zeigten sich gute Ergebnisse im Sprachverständis. Da jede monopolare elektrische Therapie, so auch die Kardioversion, ein CI schädigen kann, sollte die Nutzung von monopolarem Strom möglichst bei CI-Patienten vermieden werden. Falls dies dennoch notwendig wird, empfehlen wir dringend den Sound-Prozessor während der Behandlung abzunehmen. Kommt es trotz dieser Vorsichtsmaßnahmen zu Fehlfunktionen des Implantats, ist die Therapie der Wahl die Reimplantation.
Abstract
Cochlear implantation has become the standard procedure for the treatment of severe to profound hearing loss, even in patients with underlying diseases. We report the case of a CI patient who underwent cardiac defibrillation, following which he reported a reduction in sound quality, a worsening of tinnitus, as well as headaches. An integrity test showed multiple electrode anomalies which could not be directly attributed to the cardioversion. We performed explantation and reimplantation of the CI. During the course of rehabilitation, the patient showed good results in speech comprehension. Since cardioversion, like any electrical monopolar treatment, may damage CI, it should be avoided in CI patients wherever possible. If it cannot be avoided, we strongly recommend removing the sound processor during treatment sessions. When device-related problems occur, the treatment of choice is reimplantation.
