Zusammenfassung

Hintergrund

Chondrosarkome sind seltene Tumoren der Kopf-Hals-Region. Im Bereich des Larynx stellt das Chondrosarkom jedoch das häufigste nichtepitheliale Malignom dar.

Material und Methoden

Zwischen 1999 und Februar 2010 behandelten wir 6 Patienten mit Larynxchondrosarkom. Es handelte sich um 2 weibliche und 4 männliche Patienten im Alter zwischen 54 und 82 Jahren.

Ergebnisse

Eine 82-jährige Patientin starb 3 Monate nach Diagnosestellung und Nottracheotomie an internistischen Nebenerkrankungen. Bei einem ebenfalls 82-jährigen Patienten war auf Wunsch des Patienten eine primäre Radiatio eingeleitet worden. Bei den übrigen Patienten führten wir in 3 Fällen eine modifizierte Hemilaryngektomie und einmal eine Laryngektomie (LE) durch. Bei diesen Patienten besteht 50, 85, 87 und 95 Monate nach initialer Operation kein Anhalt für ein Rezidiv. Lymphknoten- oder Fernmetastasen fanden wir bei keinem Patienten.

Schlussfolgerung

Chondrosarkome des Larynx wachsen in der Regel langsam. Metastasen werden nach aktuellen Literaturangaben bei weniger als 3% der Patienten beobachtet. Die komplette Resektion stellt die Therapie der Wahl dar. Hierbei ist eine funktionserhaltende Chirurgie anzustreben.

Abstract

Background

Chondrosarcomas are rare tumors of the head and neck. Nevertheless, they display the most common non-epithelial malignancy of the larynx.

Materials and Methods

Between 1999 and February 2010 we treated six patients with laryngeal chondrosarcoma. The group included two female and four male patients ranging in age from 54 to 82 years.

Results

An 82-year-old female patient died 3 months after diagnosis and tracheostomy due to other underlying diseases. An 82-year-old male patient underwent primary radiation therapy. In the other patients, we performed a modified hemilaryngektomy in three cases and a laryngectomy in one. In those four cases, there were no signs of recurrent disease 50, 85, 87 and 95 months after surgery, respectively. There were no local or distant metastases.

Conclusions

Chondrosarcomas of the larynx are slow growing neoplasms. Metastases occur in less than 3% of cases. Complete resection is the therapy of choice. Function-preserving surgical approaches should be favoured.
