Zusammenfassung
Ein 32-jähriger Taucher bemerkte wiederholt nach dem Tauchgang einen starken Druckschmerz in der rechten Gesichtshälfte und eine mehrtägige Hypästhesie der rechten Gesichtsseite im Gebiet der N. infraorbitalis. Zudem berichtete er von einer Nasenatmungsbehinderung und von Problemen mit dem Druckausgleich. Es bestanden eine Septumdeviation nach links sowie in der Computertomographie (CT) der Nasennebenhöhlen (NNH) das Bild einer chronischen Pansinusitis rechts mit totaler Verlegung der rechten Stirnhöhle, subtotal der rechten Ethmoidzellen und subtotal der rechten Kieferhöhle. Nach erfolgreichem operativem Eingriff (Septumplastik und Operation der NNH – „functional endoscopic sinus surgery“, FESS) konnte der Patient nach Abschluss der Heilungsphase wieder beschwerdefrei tauchen.
Abstract
A 32-year-old diver repeatedly reported pain in the right side of the face after diving. He felt strong pain on pressure over the right face combined with hypaesthesia of the infraorbital nerve. In addition, he reported nasal breathing obstruction and problems regulating pressure in the middle ear. ENT consultation revealed a septal deviation to the right side of the nasal cavity. CT diagnosis showed the typical picture of chronic pansinusitis with total transfer of the right frontal sinus, subtotal transfer of the right ethmoid and subtotal transfer of the right maxillary sinus. Following successful functional sinus surgery (FESS), combined with septal surgery, the patient was able to dive again symptom-free.
