Zusammenfassung
Eine ernst zu nehmende Folge larynxchirurgischer Eingriffe ist ein Kehlkopfödem. In einer Studie haben wir überprüft, ob sich bei der Thyreoplastik Typ I nach Isshiki durch eine einmalige intraoperative systemische Prednisolongabe die Häufigkeit eines postoperativen Larynxödems vermindern lässt. In einem Zeitraum von 2 Jahren wurde bei 10 Patienten eine einseitige Thyreoplastik Typ I durchgeführt. Das Patientengut umfasste 7 Männer und 3 Frauen im Alter von 19–60 Jahren, wobei das Durchschnittsalter bei 48 Jahren lag. Bei 6 Patienten haben wir intraoperativ 250 mg Prednisolon i.v. appliziert, während bei 4 Patienten keine Glukokortikoide verabreicht wurden. Für die Beurteilung wurde der Larynxbefund vom 1. und 2. postoperativen Tag herangezogen. Von den 6 Patienten mit i.v.-Applikation von Prednisolon war bei 5 keine postoperative Larynxschwellung zu erkennen. Lediglich 1 Patient wies ein geringgradig ausgeprägtes Aryödem auf. Bei allen 4 Patienten, bei denen intraoperativ keine Glukokortikoide appliziert wurden, zeigte sich postoperativ ein Aryödem. Zur Prävention eines postoperativen Larynxödems erscheint uns eine einmalige intraoperative Applikation von 250 mg Prednisolon i.v. bei Erwachsenen sinnvoll.
Abstract
Laryngeal edema is considered a postoperative problem in phonosurgery. In a prospective study we examined if a single intraoperative application of prednisolon can decrease the incidence of postoperative laryngeal edema after Isshiki type I thyroplasty. We examined ten patients undergoing unilateral type I thyroplasty [seven men and three women, age range: 19–60 years (average: 48 years)]. In six patients we administered 250 mg prednisolon i.v. during surgery. In four patients no steroids were given at all. On the 1st and 2nd postoperative day, the larynx was examined in a clinical setting. Five of six patients who received intraoperative steroid medication had no postoperative laryngeal edema. Only in one of those patients we examined a small edema of the arytenoid region. In all four patients without steroid medication a postoperative edema of the ipsilateral arytenoid hump was seen. Thus, intraoperative intravenous steroid administration seems to prevent, or at least reduce, postoperative laryngeal edema. Only in one of those patients we examined a small edema of the arytenoid region.
